We will arrange a visit with you or your family, friend or representative at your home or in hospital, at a convenient date and time, to discuss your needs and how you want to receive your care.
Once you are satisfied, we will draw up your support plan and provide you with a copy.
Once you are happy with your support plan, we will agree a start day, and arrange to introduce to your carer to you at home.
Your carers will discuss your care with you daily, and ensure your care is delivered as planned and agreed.
We will provide a documentation folder, which your carers will use to document their attendance and completed tasks.
You will also be able to communicate with us through the client section of your folder, to tell us your views, add or reduce any service as you choose. We will update our records to update your care plan and reflect the changes you have made.
Also, our care coordinator will review care needs with you yearly and revise your care plan accordingly.
